1. I understand all the benefits and choices of the procedure that my pet is going to have.

2. I am agreeing that my pet has the treatments registered, according to the diagnose.

3. I had informed completely and correctly the clinical history present and past during my talk to the clinical staff of my pet. I know that until today, my pet hasn’t showed any allergy to any of the medications that the clinic will use in this case. I informed if my pet is taking any medication that can make a difference on the treatment my pet is going to have. I will inform as well if my pet is pregnant or nursing.

4. I confirm that, in case that my pet had receive a treatment in another center, I will give details of the kind of treatment that my pet received.

5. I authorize, if necessary, any additional exams, tests, interconsult or consult if is to help with the diagnose, treatment or procedure of my pet’s condition.

6. I am agreeing of any picture, filming, before, during and after any treatment with the only purpose of educating and making awareness to other pet owners, and to complete the clinical chart of the pet.

7. I give my consent to Hacienda Urbana to use the pictures and media taken to my pet for the publicity, academic and research without violate the privacy of my pet and client. And the confidentiality.

8. I accept and authorize that any local or general anesthesia is given to my pet for the best of the physical and psychological health of my pet, the safety of Hacienda Urbana staff and the comfort of the procedures that my pet is going to have on the clinic. And that I will cover the expenses of this extra education given to my pet.

9. I am agreeing that if I want to reach the desired results for my pet’s treatment, I have to be engaged with the indications and care given by my doctor in every case.

10. I am engaged with the instructions given by Hacienda Urbana’s medical staff before and after any treatment given.

11. I here declare that I won’t take any legal or physical action for this company, or its employees, or members, or involved with the company at that time, or in the future, under any condition, and that I won’t publish any content under any platform, or by any channel, and that any situation will be informed internally to the company in the best friendly and respectful possible way.
